Dr. M’s Women and Children First Podcast #63 – David Clarke, M.D. – Pain, Psychology and Trauma

This weeks guest is Dr. David D. Clarke. Dr. Clarke is President of the Psychophysiologic Disorders Association where he teaches the science of the mind body connection as it relates to GI related diseases that do not fit a traditional diagnostic nor pathological framework. For three plus decades, he conducted detailed interviews with over 7000 people whose symptoms were not explained by diagnostic testing, but were significantly affecting their lives in a negative way. He realized that these individuals often suffered from severe traumas in childhood that built the foundation of current health struggles. We dive into his work and the successes built upon the recognition of, intervention for and resolution related to the issues and physiological manifestations of disease.

Dr. Clarke graduated from Williams College with honors before attending the University of Connecticut School of Medicine where he received his medical degree and the Mosby Award for Clinical Excellence.  He completed internship and residency in Internal Medicine and fellowship in Gastroenterology at Harbor/UCLA Medical Center in Los Angeles. Dr. Clarke has lectured extensively on Psychophysiologic Disorders to specialists and the public across North America and Europe. He has appeared on syndicated broadcasts hosted by Rosie O’Donnell, by Montel Williams and by Michael Roizen(author of You: The Owner’s Manual).

He is the Author of “They Can’t Find Anything Wrong” and co-author of “A Diagnostic Guide for Psychophysiological Disorders” and Psychophysiologic Disorders”.

Literature Review
1) Pericytes are critical cells that sit around the tiny blood vessels called capillaries of the body especially in the brain. They are involved in regulating blood vessel activity. They communicate directly with the endothelial cells making them super important in the activity of the blood vessel and metabolism at the local tissue interface. Delaying Gratification and Dopamine
When we think of our current societal view on raising children, we have a conundrum. We want our children to experience the world as it is progressing through a technological viewpoint, a growth mentality. What does that mean? . When technology enters the learning process, we progress at log rates. We can process more information and incorporate it into our memory. For a simple example, let’s look back in time. When I was in school, I had to go to the library and search through the library’s archaic dewey decimal system to find a book to reference for information for a project or assignment. Time wasting to say the least compared to the internet age. It took forever to accumulate old data that was in a book. No real time action…Also a mini literature review and a recipe of the week.

1) From Cell Host and Microbe – “The microbiomes of cesarean-born infants differ from vaginally delivered infants and are associated with increased disease risks. Vaginal microbiota transfer (VMT) to newborns may reverse C-section-related microbiome disturbances. Here, we evaluated the effect of VMT by exposing newborns to maternal vaginal fluids and assessing neurodevelopment, as well as the fecal microbiota and metabolome. Sixty-eight cesarean-delivered infants were randomly assigned a VMT or saline gauze intervention immediately after delivery in a triple-blind manner…… and much more about the cutting edge science of today. Also a piece on napping followed by the recipe of the week.

What is it about Attention Deficit and Hyperactivity that we see a disease needing a drug to mitigate the problem versus first looking at the other possible root causes? (Some children do have a genetic or epigenetic condition that is not based on the following discussion.) I asked Dr. Sandy Newmark this question, here is his response: “I read Scattered minds a long time ago and thought it was a fantastic book. I agree with much of what he, and you, say. I do think he has a more of a narrow approach to etiology than I would support. I have many kids with clear ADHD who come from loving homes with 2 parents and little trauma history. There are genetic, epigenetic, and environmental factors that are also important in etiology, and I would not want those to be overlooked.”

The fundamentals are these: Age related changes that lead to neuronal loss and cognitive decline are related to loss or reduction of myokine release, systemic inflammation, insulin resistance, reactive oxygen induced mitochondrial stress, DNA mutations and poor protein intake.
First, exercise releases chemicals called myokines which are cell signaling molecules that have the job of telling other cells what to do via changes in gene expression, protein transcription and much more. The prototypical change is the increase of a substance called brain derived neurotrophic factor, BDNF, for short. BDNF is critical for brain cell mitochondrial biogenesis. BDNF promotes many developmental functions in the brain, including neuronal cell survival, differentiation, migration, dendritic arborization, and synaptic plasticity. Regular exercise promotes a progressive increase in BDNF protein for up to at least 3 mo………

Dr. M’s Women and Children First Podcast #36 Nancy O’Hara – MD, MPH – PANS/CANS/PANDAS

This weeks guest is Dr. Nancy O’Hara, a leading expert in the field of basal ganglia encephalitis and pediatric abrupt onset autoimmune neuropsychiatric disorders. Dr. O’Hara graduated with highest honors from Bryn Mawr College and was part of the Alpha Omega Alpha Honor Society from the University of Pennsylvania School of Medicine. She earned her Master’s degree in Public Health from the University of Pittsburgh, and completed her residency, chief residency and general pediatric fellowship at the University of Pittsburgh.
Before she began her medical career, Dr. O’Hara taught children with autism. From 1993 to 1998, Dr O’Hara was a practicing general pediatrician and partner in a group practice. In 1999, she began her consultative, integrative practice for children with special needs, dedicating her practice to treating children with neurodevelopmental disorders, ADHD, PANS/PANDAS and BGE, OCD, Lyme, and Autism Spectrum Disorder.
She recently published a book, Demystifying PANS/PANDAS, a desktop reference on basal ganglia encephalitis. She spends a large bucket of her time training and teaching other clinicians how to treat these complex disorders in children.
This is a very in-depth conversation that is a full pathway of cause to effect and resolution.
